Insurance Plan Participation

We participate in most insurance plans. As a service to our patients we do not charge for filing claims. However, you are responsible for providing the office with the correct information and notifying us of any changes in coverage. Our office will not be responsible for incorrect filing due to incorrect information.  We will work with you in obtaining all necessary information for submission of claims.

All co-pays are to be paid by the patient at time of service.
